To obtain a permit under this article applicants shall supply to the chief of police
the following information: Name, social security number, and date of birth of applicant,
and if a corporation, the names, social security numbers and dates of birth of all
officers and directors who shall be designated as to their office; address of applicant,
and, in addition, if a corporation, the addresses of all officers and directors; names
of employees, social security numbers and dates of birth and their addresses; criminal
convictions, other than traffic offenses of applicants, officers, directors, and employees;
street address of place from which business is to be conducted, and the location where
items described in section 15-126 shall be kept during the time period set forth in section 15-129; a recent photograph of applicant, officers, directors and employees; the days of
the week and hours of the day during which business will be conducted. A willful providing
of false information shall constitute a violation of this article.
